Feature summary

Until now we (in backend code) automatically create a patch test entity at 72hr before the appointment when the involved treatment requires a patch test.

We're changing this behaviour and now Frontend code is responsible to send the date of patch test appointment after partner selection.

This selection will be mandatory so we need to accept a new parameter (patch_test_requested_at) during appointment book and multi_book phases, based on that we'll calculate patch test availability and create relative entity.

We need also to accept another parameter (patch_test_performed_at) to create already done patch test.

• Allow appointments controller book and multi_book actions to receive and use patch_test_requested_at xor patch_test_performed_at
• Disinct "from web" and "by venue" patch test creation flows
